Focus of Child & Adolescent Wraparound Facilitation
The Child and Adolescent Wraparound Facilitation (WRAP) program aims to engage youth experiencing mental health or behavioral challenges through a collaborative effort. The program focuses on empowering families to build a support team, including friends, family, therapists, and caseworkers, to work towards common goals. The ultimate objective is to create and implement a cohesive plan that addresses the family's needs, utilizing strengths, resilience, and hope.
Where you Fit In:
The Wraparound Facilitator collaborates with families to develop a comprehensive family story and assemble a team that constructs a care plan tailored to the family's requirements. This role acts as a liaison to coordinate partnerships aligned with the care plan for each assigned family. The Wraparound Facilitator oversees the financial and operational aspects of all providers involved in the care plan, ensuring progress and adherence to guidelines.
What to Expect:
Our Wraparound Facilitator dedicates a significant portion of their services to community engagement, offering flexibility in scheduling and locations. Some evening availability is necessary to accommodate both youth and caregivers for team meetings. Am I Qualified?
  • Bachelor's degree in human services, psychology, or education, with at least two years of experience in relevant fields.
  • Wraparound DMHA Certification required (obtainable during employment).
  • Valid Indiana driver's license meeting Eskenazi Health's standards; vehicle registration and insurance are mandatory.
  • Clear Department of Child Services background check essential.
Essential Functions and Responsibilities
  • Conduct assessments of youth/family needs via family story/timeline development.
  • Guide families through the wraparound process.
  • Implement the four phases of wraparound approach.
  • Facilitate child and family team meetings regularly.
  • Maintain accurate documentation of care plan progress.
