Key Responsibilities:
  • Deliver skilled nursing services at the patient's home as outlined in the treatment plan
  • Assess and communicate significant findings promptly to relevant parties
  • Conduct thorough pain assessments and educate patients and families on pain management
  • Demonstrate knowledge of medications and their appropriate administration
  • Ensure patient safety and minimize healthcare errors
  • Provide necessary patient care education
  • Collaborate effectively with the healthcare team
  • Adhere to Agency policies and procedures
  • Participate in mandated activities and in-service education
  • Address patient care needs based on standards and age requirements
  • Respect and support cultural practices within ethical boundaries
  • Complete documentation accurately and per policy
  • Participate in interdisciplinary team conferences
Requirements:
  • Active LPN/LVN license in good standing
  • Relevant LPN/LVN experience in acute care or home care settings
  • Graduate of an accredited nursing program
  • Prior experience in home healthcare preferred
  • Valid health certificate, physical examination, and TB testing
  • Effective verbal and written communication skills
