Responsibilities
  • Patient Assessment and Care: Utilize the nursing process to assess, plan, implement, and evaluate patient care, ensuring the highest level of quality and safety.
  • Communication and Collaboration: Perform focused interviews to identify specific patient needs, assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition, and collect, analyze, and interpret data and information from healthcare members.
  • Documentation and Record-Keeping: Accurately document actual and/or potential nursing diagnoses, ensuring compliance with regulatory requirements and company policies.
  • Other Duties: Perform other LPN duties as assigned, including but not limited to, administering medications, performing wound care, and providing education to patients and families.
Qualifications
  • Current LPN License: Possess a current Licensed Practical Nurse (LPN) license in the state in which you practice.
  • Health Certificate: Hold a current Health Certificate (per facility).
  • PPD or Chest X-Ray: Have a current PPD or Chest X-Ray.
  • BLS Card: Possess a current BLS card.
  • Experience: One year prior LPN experience preferred.
