Job Title
We are in search of a Social Worker AIM to become part of our team. In this role, you will directly report to the Director of Care Management AIM. Your responsibilities will involve collaborating with patients, their families, PCPs, AIM, and other healthcare experts to identify and address social and mental stressors that exacerbate symptoms of serious illnesses. You will assist and educate patients and families in comprehending and fulfilling advance care plans aligned with their preferences. Additionally, you will create and implement care plans, handle crisis situations by providing essential resources or referrals, and offer interventions for both individuals and families when necessary.
About You
- Master's degree in Social Work from an accredited institution
- Ability to obtain multistate licensing as an LMSW or LCSW
- Minimum of 2 years of previous social work experience in a healthcare environment, preferably in palliative care, advanced illness management, or hospice
- Familiarity with palliative care, hospice principles, and the challenges of end-of-life processes
- Understanding of the effects of terminal diagnoses and anticipatory grief on psychological well-being, family dynamics, and community interactions, along with knowledge of suitable therapeutic interventions
- Capability to establish and nurture meaningful relationships with patients, families, and peers
