Essential Functions:

  • 50% - Leadership
  • Cultivates and promotes a mission-driven culture, focusing on customer service, consistency, dignity, and accountability.
  • Directs the team, collaborates with leadership, and supports department goals.
  • Manages, trains, and sets goals for the team.
  • Provides guidance on Utilization Management and guidelines.
  • Leads Interdisciplinary Team (IDT) meetings.
  • 45% - Program Oversight
  • Coordinates center activities, care planning, and safety of participants.
  • Participates in intake and assessment meetings.
  • Acts as a liaison between disciplines and staff.
  • Collaborates with the primary care physician staff on care plans.
  • Manages the budget and communicates with leadership.
  • Engages with administration for program development.
  • Ensures documentation accuracy in medical records.

All you'll need:

Bachelor's degree in health care administration, business administration, or related field required.

5 years of supervisory/managerial experience in a healthcare setting required, preferably in a PACE environment.

3 years of experience with older adult populations required.

Equivalent combination of education and experience may also be qualifying.

Preferred Qualifications:

Master's degree in health care administration, business administration, or related field.

Experience overseeing clinical departments and guiding clinical decisions.

Required Licensure / Certifications:

CPR and First-Aid Certification required or must be obtained within six (6) months of date of hire.
