RN Stroke Coordinator

JOB SUMMARY


Provides leadership and vision for the Stroke Program in terms of operations, services & patient care, consistent with best practices & evidence based care.

Serves as an active member of University Hospitals Core Stroke Team overseeing coordinated care of the stroke patient in terms of health prevention, health promotion, and rehabilitation.

Coordinates care for Stroke team patients- Data collection, rounding, staff education, survey readiness, reports and PI projects are all involved.

Must articulate well and be knowledgeable of PowerPoint and working with Excel spread sheets.

This is a Full time, benefit-eligible (paid Medical) position that will be scheduled primarily on Day shift with Call.

Education and Experience

  • NIHSS & BLS (AHA or ARC) required.
  • BSN required.
  • Three (3) years Clinical Practice experience required; strong Neuro background preferred.
  • Stroke experience required.
Job Duties

Identifies, tracks, & supports all inpatient stroke care provided in University Hospitals & reports findings/results on a monthly basis to Stroke Committee.

Coordinates and facilitates provision of educational opportunities for staff, physicians, patients, external providers of care, and the community. Identifies and pursues opportunities to provide information and/or services to the community to promote UTSW and the Stroke Program. Identifies relevant areas for nursing research, initiates projects and participates on research team outcomes.

Leads and directs achievement in the provision of quality care to the stroke patient population. Oversees development of appropriate policies and procedures required for the provision of effective and efficient stroke care. Analyzes data & coordinates implementation of practice changes as needed. In conjunction with Clinical Education and Department Managers, coordinates education and verifies competence of staff performing NIHSS.

Adheres to appropriate hospital and/or Department of Nursing policies, procedures, clinical pathways and nationally accepted standards of nursing practice.

Incorporates ANA standards, professional practice model (Shared governance) and care delivery model (Relationship Based Care) into the provision of safe, quality patient care.

Nursing care is based on current literature and research.


Provides timely, confidential coaching and/or counseling of nursing personnel as appropriate and follows up with accurate reports to Clinical Nurse Manager.

Assures first person reporting of events through RL Solutions. Ensures all staff are competent in entering event reports & serves as a resource when necessary. Investigates facts of event reports as able. Facilitates timely receipt of these reports for follow-up by the Clinical Nurse Manager. Notifies Nurse Manager/Risk Manager immediately of high risk situations.

Actively participates in routine & special patient care conferences.

Promotes and enthusiastically supports professional growth of staff by providing equal opportunities for participation in unit based activities, patient care/unit decisions, hospital committees, and/or continuing education.

Collects information and maintains records regarding performance and contributions on an on-going basis throughout the fiscal year. Provides input and/or conducts annual performance reviews and orientation evaluation of personnel assigned to the unit.

Coordinates self-scheduling to maintain appropriate coverage for unit needs.

Maintains Kronos records on a daily basis, monitoring for trends that require intervention ie., late clock-ins, incremental overtime, missed punches.

Assumes delegated responsibility for inventory control of equipment and supplies to ensure safe and effective patient care. Facilities timely return of rented or borrowed specialty equipment. Solicits staff input regarding equipment and supply needs for the unit.

Assists with staff meetings, special projects and the development, implementation & evaluation of performance improvement activities. Communicates data and solicits input from staff regarding performance and quality improvement i.e. patient safety, infection control. Actively participates on hospital work groups. Performs other duties as assigned.

WORKING CONDITIONS
Work is primarily performed in a hospital unit/environment. The incumbent may be exposed to body fluids and infectious diseases. Pass pre-employment physical exam including colorblind testing.


Any qualifications to be considered as equivalents in lieu of stated minimums require prior approval of the Vice President for Human Resources Administration, or his/her designee.


This position is security-sensitive and subject to Texas Education Code §51.215, which authorizes UT Southwestern to obtain criminal history record information.
