- Provide physical therapy services to patients based on a prescribed plan of care.
- Assess and evaluate therapeutic/rehabilitative/functional status, and participate in developing the overall care plan, including evaluating the home environment.
- Evaluate muscle strength, mobility, gait, and potential for rehabilitation.
- Direct physical therapy treatment and educate patients/families on therapeutic appliance use and care.
- Determine priority needs for physical therapy.
- Report patient responses to treatment or changes in condition to the physician.
- Initiate physical therapy programs and provide instructions to other personnel/family members involved in patient care.
- Train patients in the use of prosthetic devices.
- Identify additional home health service needs for patients and refer when necessary.
- Submit all clinical documentation in a timely manner.
- Participate in discharge planning and prepare progress summaries.
- Offer physical therapy consultations to families/caregivers and provide in-service education programs as needed.
- Participate in peer consultation and supervise physical therapy assistants.
POSITION QUALIFICATIONS:
- Must have a degree in physical therapy from an accredited program.
- Valid state physical therapy license.
- Minimum of two years experience as a physical therapist. Community/home health experience is a plus.

Company Fleet Vehicles Available: Equipped with a gas card, maintenance, repairs, and insurance coverage for your convenience and efficiency.
Job Type: Full-time
Pay: $105.00 per hour
