Responsibilities:

  • Coordinate care provided by the team
  • Communicate with all disciplines involved in patient care
  • Ensure all visits are authorized and comply with physician orders
  • Review patient visits weekly
  • Manage schedule changes and communicate care issues

Requirements:

  • BLS (Basic Life Support) certification
  • Active RN (Registered Nurse) License
  • Valid Driver's License in Florida

Education and Experience:

  • Associate's or Diploma in Nursing required
  • Bachelor's in Nursing preferred
  • Minimum 1 year of RN or LPN experience
  • Experience in home care is a plus
