  • Home visits in assigned service area: Dunedin, Palm Harbor, Tarpon Springs, Oldsmar, and surrounding areas

Responsibilities:

  • Evaluate and develop specific treatment programs for individual patients based on their needs and in accordance with the principles and practices of physical therapy.

  • Provide the appropriate level of clinical direction for Physical Therapy Assistants.

  • Plan, implement, manage, and evaluate the provision of both professional and ancillary home health services (direct care and case management) to ensure that all patients' needs are met and quality care provided in accordance with Federal, State, JCAHO, and program guidelines.

  • Manage the activities of the multi-disciplinary team providing care to the patient.

  • Monitor the provision of therapeutic services through written and verbal communication with all disciplines.

  • Develop, implement, and carry out a discharge plan in conjunction with the patient, family/caregiver, and members of the health care team.

  • Interpret agency policy to patients/families.

  • Review patients' visits weekly and provide a plan to Home Care Supervisor and scheduler, including anticipated weekend visits.

  • Inform scheduler of any schedule changes and communicate caseload and patient care issues to Home Care Supervisor.

Requirements:

  • Bachelor's Degree in Physical Therapy

  • Physical Therapist State of Florida License

  • 1 year Physical Therapist experience

  • Basic Life Support

  • Driver's License (Florida driver's license is required within 30 days of hire)

  • Preferred Master's degree in Physical Therapy
