Summary:

The FirstAssistant is responsible for assisting the surgeon in performanceof the surgical procedure.

Focus/Purpose:

Certified and orLicensed Surgical First Assistants (SEA) are an integral part ofthe team of medical practitioners providing surgical care topatients in a variety of settings. In order that surgery mayproceed smoothly the SFA is a specifically trained individual whoprovides assisting services for the surgeon in both the preparationfor and performance of surgery and anticipates the needs of thesurgeons. These services include, but are not limited to, tissuehandling, wound retraction/exposure, positioning, providinghemostasis and Suturing of tissue, as directed.

Requirements:

Current Certified Nurse of the Operating Room (CNOR)orCertified Surgical Technologist (CST) preferred.

Documented completion of an approved first assistingcourse required, certification (CRNFA, or CFA) preferred

BLS completed, ACLS preferred

Educational Requirements: Must hold Currentcertification as a Surgical First Assistant (CSFA, CSA, SA
•C, CSAor RNFA) or a license as a Mid-level provider or Medical provider(PA
•C, AR p, MD or DO)

Responsibilities:

  • Be able to perform all responsibilities of asurgical technologist, if need and trainedappropriately
  • Determine specific equipmentneeded per procedure Review preference card to confirm procedureand special needs.
  • Select and place Ofx-raysor initiate CD for reference
  • Participate inAnesthesia time out.
  • Assist with gettingpatient in room andpositioned
    • Untie gownso OT to lay on knots
    • Assist With movingpatient to OR bed from stretcher
    • Assist patientand anesthesia during spinaVepidural
    • Assistanesthesia When placing tube and IV Whenappropriate
    • Assist With all forms Ofpositioning: supine, lateral, beach chair, tuck arms, placefootboard when appropriate, etc.
    • Placetourniquets, Foleys, bear hugger, SCDS and bovie pad when necessaryand ifappropriate
  • Confirmprocedure with surgeon and Operating room team per theconsent
  • Drape patient withinsurgeon's guidelines
  • Provideretraction of tissue and Organs for optimal visualization withregard to tissue type and appropriate retraction instrument and/ortechnique
  • Assist in maintaininghemostasis by direct pressure, use and application of appropriatesurgical instrument for the task, placement of ties, placement ofsuture ligatures, application Of chemical hemostatic agents, OrOther measures as directed by the surgeon
  • Use electrocautery mono and bi-polar persurgeon's directive
  • Clamp, ligate,and cut tissue per surgeon's directive
  • Placinghemoclips on vessels per surgeon'sdirective
  • Assist surgeon in use of the Robot(DaVinci, MAZOR, MAKO, etc.) place trocars during laparoscopic orrobot assisted surgery per surgeon'sdirective
  • Fire stapling devices during surgicalprocedure per surgeon's directive
  • Harvestsaphenous vein, including skin incision per surgeon'sdirective
  • Dissect common femoral artery andbifurcate per surgeon's directive
  • Maintainintegrity of sterile field
  • Close all woundlayers (facia, subcutaneous and skin) as per surgeon'sdirective
  • Insert drainage tubes persurgeon's directive
  • Place a screw ormanipulate a tool when manipulation and positioning of the hardwaremust be held in such a specific position that that particularmaneuver can only be accomplished by the orthopedicsurgeon
  • Prepare a tendon for grafting persurgeon's directive
  • Infiltrate local medicationper the surgeon's directive
  • Apply wounddressings and if necessary application of cast, splints, and otherdevices
  • Assist with resuscitation of patientduring cardiac alTest or other life-threatening events in theoperating room
  • Perform any other duties orprocedures incident to the surgical procedure deemed necessary andas directed by the surgeon
  • Help withtransferring patient from OR table tostretcher
  • Prepare room for the nextcase
