Job Summary:

As a Community Support Specialist, your role involves providing support and engaging in activities that are part of a structured treatment plan. This plan aims to facilitate an individual's seamless adjustment to community living.

You will be based in the community, which includes visiting clients' homes, clinics, and various locations within the community as part of your assigned caseload.

We are seeking a candidate who is deeply committed to promoting recovery and enhancing resilience. The ideal candidate should possess experience in evidence-based interventions, along with proficient communication and collaboration skills.

Key Responsibilities:
  • Provide mental health services in the community to children, adolescents, and adults
  • Collaborate with clients, families, and stakeholders to develop individualized treatment plans
  • Implement evidence-based practices and treatments
  • Coordinate care with other healthcare providers and organizations
  • Maintain accurate client records
  • Participate in staff training and development
  • Contribute to quality improvement initiatives
  • Facilitate group sessions for treatment goals

Education and/or Experience Qualifications:
Bachelor's degree in Psychology, Social Work, or a related fieldOR Bachelor's degree in an unrelated field with 2 years of relevant experienceOR equivalent combination of education and experienceOR Associate's degree in Behavioral Health SupportOR 4 years of qualifying experience*Qualifying experience includes providing services to individuals with mental health, substance use, or developmental disorders, offering rehabilitation/habilitation services, teaching coping skills, and more.

Employment Requirements:
  • Successful background check
  • Completion of new hire orientation within 30 days
  • Completion of all required trainings
  • Thorough knowledge of program policies and procedures
  • Valid driver's license with a good driving record
  • Age requirement of 21+ to operate company vehicle, 23+ for a 15-passenger van
  • Reliable transportation with valid auto insurance

Physical Requirements:

Work involves exerting minimal force occasionally and primarily involves sitting, with some brief periods of walking or standing. The role meets ADA criteria for sedentary work.
