Job Description

This position serves as the primary discharge planner for individuals who have been hospitalized for an extended period. This role will also require the ability to link individuals to appreciate services and resources in the community, complete assessments and referrals, and work collaboratively from a multi-disciplinary recovery perspective.

Preferred Qualifications

In addition to the State qualifications, preference will be given to candidates who possess some or all of the following:

· Must have a Master's in Social Work (MSW) , License-eligible

·Experience working with persons with Mental Illness and/or Developmental Disability

Employment Requirements
