Avenir Memory Care at Little Rock is a premier Memory Care community with an established reputation. We are looking for LPN's to join our team that want to make a difference in the lives of our residents and family members, and who are looking for a work "home" We are looking for a part-time LPN and we also are looking for PRN employees to pick up at least 2 shifts a month Night Shift - 7 pm- 7 am. Shift Differential for nights and weekends

Position Summary:

Provides direct nursing care to residents and delegates and supervises the care provided b y Quality of Life Specialist (CNA's) Administers medication, performs treatments and p rocedures, obtains specimens as needed, completes documentation as required.

Essential Job Function:

  1. Documen t resident status as per policy
  2. Provide daily supervision to assigned staff and directly provide nursing care to R esidents as assigned.
  3. Participate in the admission of new residents.
  4. Assist residents with Activities of Daily Living (ADL), family/social activities and actively promote independent life- style of residents.
  5. Respond to residents call system promptly and courteously
  6. Responsible for identification of resident medical and behavioral changes and the development and monitoring of care plans and re-assessment of each resident .
  7. Provide daily report to the Director of Health Services concerning facility c onditions, personnel performance, resident conditions, and needs for nursing supplies.
  8. Provide nursing care according to physician's orders and in conformance with recognized nursing practice, established standards, and administrative policies.
  9. Ensure preventative, therapeutic and rehabilitative services to residents and assists in the coordination of nursing care with care rendered by other disciplines .
  10. Make rounds with physicians, assist in examinations and/or treatment of residents and carries out order.
  11. Assess resident ad notify attending physician and responsible party in the event of a change in the resident's condition.
  12. Accurately administer prescribed medications, supervise the continuity of nursing care, and respond to changes in the resident's condition.
  13. Provide daily supervision and leadership to assigned staff and other designated personnel in meeting the resident's need. Demonstrate good decision making skills , provide written evaluation and oral counseling of assigned staff.
  14. Assess and monitor complex resident health status and determine the appropriate nursing intervention.
  15. Develop and monitor the ongoing plan of care with resident and initiate corrective action as necessary.
  16. Orient residents and staff to emergency procedures and respond to all emergency situations.
  17. Complete and sign incident reports and provide follow-up as required.
  18. Assure that all resident records are accurate, complete and all mandatory forms are on file.
  19. Through familiarity and application facility policies and procedures, assure that nursing care is practiced in full accordance with regulatory requirements, facility quality standards, and residen t rights.
  20. Monitor resident environment to ensure all safety and sanitary regulation and quality standards are met or exceeded.
