Contribute directly to breaking the cycle of homelessness for women through comprehensive supportive services offered at DWC's drop-in Day Center and in the community The Case Manager - Day Center delivers services with a trauma-informed approach and is a patient supporter who actively listens and maintains appropriate boundaries. This position connects clients with services and resources, serves as an advocate and collaborates across departments to meet the immediate and long-term needs of each woman. Reporting directly to the Supervisor – Programs, Day Center Case Management, this position works in close collaboration with the Clinical and Health & Wellness teams to empower women to achieve and maintain personal stability.

Essential Functions Provide trauma-informed case management services to women experiencing homelessness Create individual service plans with clients that address their needs and goals; identify and provide needed resources and referrals Complete Trauma Recovery Center (TRC) intakes for individuals who have experienced trauma related to sexual assault, domestic violence, and human trafficking crimes Administer the Coordinated Entry System Housing survey (CES) as needed and maintain accurate records, provide follow up for clients matched to permanent supportive housing Assist clients in securing and maintaining housing, basic needs, medical and mental health services (including substance abuse treatment when necessary), job training, and public benefits Work to effectively meet clients' needs and resolve individual barriers through follow-up, advocacy, and collaboration with DWC staff and other community service providers Maintain accurate and timely records of activities and services provided to clients in the Homeless Management Information System (HMIS) database system, case management notes, and other tracking tools Contribute to the enhancement of policies and procedures to ensure best practices Facilitate workshops that address basic needs such as, but not limited to, legal needs, housing, transportation and benefits Develop and maintain relationships with clients, staff and local service providers, participate in community meetings pertaining to services benefitting clients, serve as a representative of DWC at community events Provide regular, timely written reports to supervisor and additional reports as assigned Provides effective crisis interventions in consultation with program supervisors, and establish wellness plans for clients experiencing crisis Other duties as assigned to support the mission of the organization Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.

Work Environment
This job operates in a Day Center Drop-In setting. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

While performing the duties of this job, the employee is regularly required to talk and hear. Specific vision abilities required by this job include close vision and ability to adjust focus. This would require the ability to lift files, open filing cabinets and bend or stand on a stool as necessary.

Position Type and Expected Hours of Work

This is a full-time non-exempt position. Days and hours of work are Mondays-Fridays 7:30am-4:00pm.

Required Education and Experience Education in social work/related field or relevant experience preferred Strong knowledge of the complexity of poverty and women's homelessness, mental illness, trauma, substance abuse, health issues, and older adults preferred Experience working with homelessness preferred Experience providing trauma-informed services preferred Experience working with mental health and substance abuse issues preferred Minimum two years of experience working as a Case Manager/similar position and knowledge of community resources strongly preferred Required Knowledge/Skills/Position Qualifications Excellent active listening and crisis intervention skills with the ability to exercise and model good judgment Able to solve problems by being resourceful and using creativity to assist women with removing barriers Strong interpersonal, written and oral communication skills; proven ability to communicate with diverse audiences Ability to manage multiple tasks including proficiency in balancing direct service responsibilities with effective record keeping Detail-oriented, highly organized, able to manage multiple tasks and priorities, set and meet goals and deadlines Strong administrative skills and proficiency in Microsoft Office Suite (Word, Excel, Outlook) required Current valid California state driver's license, insurance, and vehicle with good driving record Oral and written fluency in Spanish strongly preferred · Cultural humility. · Must be a CA resident and live no more than 75 miles from DWC; must be available for in-person meetings at DWC. · Must provide documentation of a negative TB test result within the last twelve months, on first day of hire. · Must provide documentation of a negative COVID-19 test result from the last seven days, on first day of hire. You will be required to be up to date on your vaccinations against COVID-19 and provide documentation. If you are fully vaccinated and not yet eligible to receive a booster dose, then you must receive the booster dose within 30 days of becoming eligible and eligibility begins 5 months after receiving final primary dose. If you are fully vaccinated and have received a booster dose, you are considered up to date with your vaccinations. Unvaccinated individuals must receive their 1st dose before their first day of hire. Partially vaccinated (for 2-dose series) individuals must receive their 2nd dose within 30 days of being hired. The second dose of a two-dose vaccine series is due 4 weeks after the primary dose. Additional Eligibility Qualifications
