We are a multispecialty medical group seeking a Primary Care Nurse Practitioner/Physician Assistant to provide full spectrum primary and preventative care.

The Physician Assistant shall be responsible to provide follow-up care to patients with diabetes and endocrine disorders with attention to medication management for glucose, blood pressure and cholesterol control; other preventive care for diabetic patients; and communication with primary care providers.
* Will develop expertise in the management of patients with diabetes and other endocrine disorders
* Demonstrates a fundamental grounding in nursing/medical practice with mastery of knowledge within a defined content area.
* Manages a caseload of patients in partnership with the healthcare team to optimize health status and coordinate care over time and across healthcare settings.

*Responsibilities:*
* Prescribe all necessary medications and treatments
* Perform comprehensive physical examinations of patients
* Develop and implement patient management policies and procedures
* Facilitate referrals to other healthcare professionals and medical facilities
* Communicate with collaborating physician or specialist regarding patient care

*Qualifications:*
* Previous experience in nursing or other medical fields
* Ability to build rapport with patients
* Ability to thrive in a fast-paced environment
* Excellent written and verbal communication skills
* Strong leadership qualities

*EDUCATION:*

*Physician Assistant*
* Masters of Science Degree, Graduate of accredited Physician Assistant Program Certification
* Current CA licensure

*EXPERIENCE PREFERRED (NOT REQUIRED):*
* Outpatient practice experience
* focuses on the primary care needs of adult and older patients.
* Experience with Electronic Medical Record
