Summary

Duties

This position is located at the Audie L. Murphy Veterans Memorial Hospital in San Antonio, TX. The Spinal Cord Injury Center Acute Rehabilitation Program Clinic Coordinator Senior Social Worker works independently as an advance practice social worker. The incumbent coordinates admissions of new spinal cord injured Veterans and Active Duty Service Members, and helps their families navigate the process of illness and injury, from hospitalization to post-care transition. The incumbent has specialized knowledge, understanding, and experience in medical social work and complex dispositions resulting from spinal cord injuries. The incumbent will work with an interdisciplinary team providing psychosocial services and facilitating post-care transitions to Veterans with highly complex health, mental health, and psychosocial needs. As such, the incumbent must possess excellent interpersonal skills and be adept in the areas of advocacy and negotiation within the context of an interconnected professional team of providers, Veterans and other stakeholders.

Duties may include but are not limited to:
-Ability to independently conduct admission assessments and psychosocial assessments to determine needs of Veterans and family members.
-Provide wellness/prevention education and will refer Veterans/families to the appropriate interdisciplinary team member for identified health education needs.
-Assesses high risk factors, acuity, and the need for services.-Ability to provide consultation/education to Veterans and families about Advance Directives.
-Document social work interventions and activities in patient's clinical record utilizing CPRS and ensure appropriate hand off when transferring of patient care.
-Responsible for assisting with preparation for Commission on Accreditation of Rehabilitation Facilities (CARF) inspections.
-Maintains data and statistical compilations to comply with the VA's policy and procedures.

Work Schedule: Monday-Friday, 7:30a.m. to 4:00p.m.

Pay: Competitive salary and regular salary increases.
Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)
Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
