POSITION SUMMARY

CLUES’ Certified Community Behavioral Health Clinic (CCBHC), Abriendo Caminos, will be providing immediate access to supportive, high-quality, culturally-responsive and integrated Behavioral Health services and pathways to wellness for the Latinx Community, regardless of insurance status or ability to pay. The CCBHC model provides CLUES with a model for sustainable Behavioral Health service provision, which translates to increased access to consistent care for wellness in the Latinx community.

CLUES’ Adult Mental Health Targeted Case Manager collaborates with Behavioral Health leadership and staff to provide integrated care within the CCBHC model. The position is responsible for promoting behavioral health services both internally within CLUES and externally within the community, while also assisting staff in areas including but not limited to quality, outcome-based design, service relevance, delivery, effectiveness, and evaluation, all in alignment with CLUES mission.

Schedule: Full-Time. Generally, Monday – Friday Office hours. Occasional/Some evening or weekend hours may be required.
Estimated Salary/Pay: $21.25 to $25.00 (depending on experience & qualifications).

ESSENTIAL DUTIES AND RESPONSIBILITIES
 
  • Provide bilingual and culturally responsive Adult Mental Health Targeted Case Management (AMH-TCM) services to help adults with Serious and Persistent Mental Illness (SPMI) gain access to medical, social, educational, vocational and other necessary services connected to the person’s mental health needs.
  • Complete required client assessments and plans in a timely manner, which includes the Functional Assessment (FA), Individual Community Support Plan (ICSP), and client crisis plans. Review and update these plans every 180 days.
  • Maintain up-to-date client documentation, including case notes and agency required reporting.
  • Based on a person-centered assessment of each client’s needs and existing supports, refer and link clients to mental health and other services, ensuring coordination of services across settings, and monitoring the delivery of services.
  • Support clients at mental health related meetings, appointments, and court appearances. Provide transportation as needed and deemed appropriate.
  • Support clients around the process of civil commitment and re-entry into the community, including monitoring client progress and making recommendations to the court, when necessary.
  • Respond promptly to client crisis situations and seek support from supervisor in high-risk situations.
  • Collaborate with Behavioral Health leadership and staff to provide integrated care within the CCBHC model.
  • Participate in individual and group supervision as required within the team and the department. Attend other meetings as required by contract or agency.
  • Support supervisor in cultivating and maintaining a positive working relationship and regular communication with contract stakeholders and service collaborators.
  • Meet goals and outcomes set for case load and in-person client contact requirements (e.g., meeting at least monthly with each client, and serving a pre-determined number of clients per month, etc.). 

General Duties
  • Promote the mission, operating values, and agency services of CLUES within the community. 
  • Maintain client confidentiality in accordance with HIPAA, agency policies and other applicable laws, regulations and ethical standards.
  • Carry out all other duties as assigned by supervisor.

QUALIFICATION REQUIREMENTS

Education, Experience & Key Abilities  
  • Candidates must meet state qualification standards for “case manager” or “mental health practitioner” to be considered for this role. Candidates must satisfy at least one of the three scenarios below: 
  • Be a mental health practitioner as defined in section 245I.04, subdivision 4;
  • Have a bachelor’s or master’s degree in one of the behavioral sciences or related fields including, but not limited to, social work, psychology, or nursing from an accredited college or university; or
  • Candidates who are not mental health practitioners or don’t have a bachelor’s degree may only be considered if one of the following applies:
    • Have 3 to 4 years of experience as a case manager associate;
    • Be a registered nurse and have a combination of specialized training in psychiatry and 3 years of work experience consisting of community interaction and involvement or community discharge planning in a mental health setting; or
    • Be qualified as a case manager under the 1998 Department of Human Service waiver provision and meet the continuing education and mentoring requirements
  • Bilingual in Spanish and English required.
  • Must have a deep understanding of Latinx cultures, values, and traditions, and demonstrate cultural responsiveness in every aspect of the role, including communication with colleagues, staff, clients, community members; creating forms and documents; hiring and training staff; and in clinical supervision and consultation.
  • Experience in case management or other direct client services preferred.
  • Must be organized, have the ability to work under pressure and stress, have attention to detail, be self-motivated, and have the ability to multi-task in a fast-paced environment.
  • Must be skilled in the process of identifying and assessing a wide range of client needs.
  • Must be comfortable discussing sensitive issues with clients and staff in an appropriate, effective manner.
  • Crisis de-escalation skills and experience preferred. 
  • Knowledgeable about local community resources and how to use those resources for the benefit of clients preferred.
Language Skills
  • Effective communication in English and Spanish, both verbally and in writing, required.
  • Ability to read, analyze, and interpret written materials, including reports, contracts, and proposals. 
  • Ability to effectively present ideas and information in an organized and effective manner, including writing reports.
  • Ability to maintain accurate documentation and respond appropriately to questions from clients, co-workers, management and external parties. 
Transportation
  • Must have access to use a reliable motor vehicle for independent transportation, valid driver’s license, proof of automotive insurance, and maintain a good driving record. Must submit to and pass annual recertification of compliance with CLUES Automobile and Transportation Policy and driving record check.
Computer Skills
  • Proficient working knowledge of email software, the Internet, databases, and common Windows-based programs, including Microsoft Office Word, Excel and PowerPoint required. 
Must be willing to attend training to advance technical skills and maintain appropriate skill levels to perform job duties.

Background check
  • Must pass a criminal background check, driving record check, and MS-604.20 background check. As a condition of continued employment, staff person may be required to submit to periodic re-verification of these background checks.
