Primary focus of practice is the independent clinical evaluation of Veterans to include panel management, and/or panel coverage for providers out on leave, and/or GAP coverage for Veterans with acute or chronic complaints. Clinical evaluation will include evaluation and management of acute and chronic conditions, prevention, and patient self-management. Comprehensive primary care will be provided to male and female Veterans.
Performs and records history and physical examinations and records data from clinic visits. Enters progress notes in CPRS and completes action required encounters in a timely manner.
Orders diagnostic tests, medications, laboratory tests as medically relevant for each patient.
Determines medical assessment and develops treatment plans according to accepted protocols, physician supervisor recommendations or standing orders.
Prescribes medications according to approved formulary, and in accordance with ILLIANA VAMC credentialing, Federal and State Law, and VHA Regulations.
Processes view alerts and all other patient notifications in a timely manner, critical alert within 48 hours, and moderate alerts within 14 days. Generally, does not allow view alerts and other notifications to exceed accumulation of more than 100. Effectively addresses abnormal alert values.
Educates patients concerning medical conditions and recommended treatments. (Specific topics include nutritional counseling, need for smoking cessation, need for periodic immunizations and health screening tests, and other preventive health measures).
Consistently demonstrates expertise in diagnosing and treating seriously ill multisymptomatic patients. Consults other specialties as appropriate for patient management, according to service agreements.
Interacts with patients and Primary Care clinic staff when there are problems between patients and members of our provider staff that interferes with the efficient operation of our clinic.
Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice.
Develops and implements an individualized plan to achieve optimal population health and disease management.
Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members.
Orders and interprets labs, tests, and diagnostic studies.
Prescribes pharmacologic and non-pharmacologic interventions.
Initiates referrals and consults to specialty services.
Provides education to patients and their families.
Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.
