Full Time Mental Health Case Manager

- Act as a liaison between therapist, the referring agency, and family of the client. Arrange family visits, coordinate transportation, and ensure clients needs are met.
- Insure communication between all components of treatment.
- Attend court hearings as deemed appropriate.
- Ensure documentation occurs according to policy.
- Attend treatment staffing with prepared report that accurately represents clients response to treatment.
- Facilitate individual and group CSP as assigned.
- Function on agency quality assurance or other committees as assigned.
- Assist with in-service training as directed by Clinical Director.
- Attend supervisory sessions with prepared agenda.
- Ongoing assessment of client needs.
- Assistance in achieving personal independence in managing basis needs as identified by the client and/or parent or guardian.
- Facilitation of further development of daily living skills, if identified by the client and/or parent or guardian.
- Coordination of the ISP/ITP, including:
- services identified in the ISP
- assistance with accessing natural support systems in the community
- linkages to formal community services and systems
- Coordination and assistance in crisis management and stabilization.
- As appropriate to the care of clients, education and training specific to the individual needs, abilities and readiness to learn.
- Mental Health or AOD interventions that address symptoms, behaviors, thought processes, etc., that assist a client in eliminating barriers to seeking or maintaining education and employment
- Activities that increase the clients capacity to positively impact the clients environment
- Adequate knowledge and ability to navigate EHR (Electronic Health Record) system.
- Assume additional duties as assigned by Clinical Director or Supervisor.
- Bachelors degree in Social Work, related field, or minimum of two years college credit plus five years of work experience in the field of Human Services or QMHS training adequate to perform job duties according to Ohio Law.
- QMHS/CDCA Certification/License or higher.
- Experience working with adults, children, and adolescents with behavioral health or substance abuse issues.
- Interact in a pleasant and professional manner with other employees, physicians, providers, patients, and patients families to promote a team effort through cooperation and respect for fellow employees.
- Organizational skills, writing, speaking clearly, answering telephone, fine dexterity, handling and gripping.
- Carrying/lifting/transferring (min 50 lb.), communicating, stooping, kneeling, squatting, reaching, standing, climbing, walking, and repetitive activity.
- Calculating, interpreting information, analyzing, assessing, evaluating, explaining, teaching, attention to detail, memory and reasoning.
- Satisfactory drug screen and criminal background check.
