Design, implement, and support optimal resource utilization, cost effectiveness and ongoing quality improvement of the medical management program.
Evaluate the patient’s response to the plan of care and provides documentation that the plan and the quality of the services offered to the patient correspond to their identified needs.
Facilitate and coordinate patient care activities to include required tests, procedures, treatment, discharge planning and community referrals.
Participate in researching, interpreting, developing, and presenting elements of educational programs for healthcare professionals and support services in the areas of case management or related clinical issues.
