Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team as a Case Manager. As a key member of our Professional Case Management team, you will be responsible for providing high-quality patient care and coordinating services to ensure optimal health outcomes.

Key Responsibilities
  • Assessment and Planning
    • Conduct thorough assessments of patients' health status and develop individualized care plans to address their unique needs.
    • Collaborate with patients, families, and healthcare teams to identify goals and priorities for care.
  • Service Coordination
    • Coordinate and facilitate access to community resources, services, and support to promote patient well-being and independence.
    • Develop and maintain relationships with healthcare providers, community agencies, and other stakeholders to ensure seamless care coordination.
  • Clinical Expertise
    • Provide direct patient care, including medication management, wound care, and other interventions as needed.
    • Stay up-to-date with the latest clinical guidelines, research, and best practices in case management and patient care.
  • Communication and Documentation
    • Communicate effectively with patients, families, and healthcare teams to ensure clear understanding of care plans and progress.
    • Maintain accurate and comprehensive documentation of patient interactions, assessments, and care plans.
  • Professional Development
    • Participate in ongoing education and training to enhance clinical skills and knowledge in case management and patient care.
    • Contribute to quality improvement initiatives and share best practices with colleagues to promote excellence in patient care.
Requirements
  • Current, unrestricted license as a Registered Nurse in the state(s) of practice.
  • BSN preferred.
  • Reliable means of transportation.
  • Current CPR certification.
  • Strong communication and interpersonal skills.
Preferred Qualifications
  • Experience in case management, home health, or other healthcare settings.
  • Knowledge of community resources and services.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and critical thinking skills.
