Job Summary: 

Provides psychosocial assessments, support, short-term counseling and social work services to cancer patients and their loved ones throughout the continuum of care including diagnosis, treatment, survivorship, palliative care, end-of-life care, and bereavement. Serves as the subject matter expert on oncology psychosocial distress screening, and leads the development and maintenance of a screening program; works with financial counselors to meet the financial, transportation, and medication needs of patients. Responsible for program development and maintenance for support groups and survivorship activities, including sharing in the responsibility for facilitation of these groups. Works collaboratively with other Oncology Care Team members to facilitate care of the patient and family across the many disciplines and professionals required for their care. 

Education: 

Degree/Diploma Obtained. Program of Study Required/Preferred Masters Social Work. Required Certifications, Licensures and Registrations Credential Primary Source Verification Required/Preferred LCSWA Licensed Clinical Social Worker Associate North Carolina Social Work Certification and Licensure Board Required. - Adaptability: Maintains regular and dependable work schedule, while being flexible enough to meet patient and program demands that fall outside traditional work hours. 

Job Specific Duties Psychosocial Assessment: 

Completes psychosocial assessment to determine survivor and family's strengths and needs relative to coping effectively with cancer diagnosis, treatment and follow-up care. Uses high risk and distress screening criteria to identify survivors and families in need of Social Work services. Therapeutic Interventions Uses a range of therapeutic and other interventions, including supportive short-term counseling, group work, and education to address issues in each phase of the illness. Direct Assistance Maintains current knowledge of available resources and provides direct assistance to meet financial, transportation, lodging, and other needs. Documentation On the day of each encounter, fully documents assessments, referrals, patient progress, and known barriers to care in the appropriate section of the Electronic Medical Record. Pre-admission, transfer, and discharge planning Collaborates with social workers and discharge planners at a variety of inpatient facilities to ensure continuity of care. Provides case management services as needed. Program Development Works with community agencies and other Cancer Center staff to develop a complete oncology survivor and co-survivor support program; This includes oversight of support groups, survivorship education and activities, and development of support materials. Participates on the LoveLine Cancer Support Committee. Is an active participant with the Patient and Family Advisory Committee, the Survivorship Committee, and other groups as requested by Cancer Center leadership. Interdisciplinary Conferences Actively participates in tumor boards and interdisciplinary conferences, providing psychosocial insight and identifying patients in need of intervention. Case Management Reviews patient charts to identify needs, provides assistance in removing barriers to care, addresses gaps in services. Analyzes data from patient interviews, chart reviews, and psychosocial assessments to determine patient's needs including direct care or referral to other departments. Appropriately triages telephone calls from the patient with problems or complaints. Maintains written and oral communication with outside agencies, referring appropriate cancer patient to these agencies for psychological, spiritual, social and financial support. Patient Advocacy and Support Takes initiative in ensuring that needed patient services are available at the Cancer Center or by referral. Works with health system and community leaders to develop programs and resources as needed Programmatic Leadership Provides programmatic and thought leadership regarding oncology patient and family needs to all management units (i.e. Scheduling and Registration, Clinical Research, Infusion Center, Radiation Oncology, Oncology Care Team, Hem/Onc Clinic). Includes assisting non-clinical staff to better understand how various providers and treatments interact to facilitate efficient care. Continuing Education Avails themselves of continuing education opportunities to remain in current field. Maintains all certifications to include LCSW, and house wide and oncology-specific social work competencies. In addition when appropriate participates in grand rounds, cancer conferences, and other clinical learning opportunities Professionalism Upholds a professional working relationship at all times and adheres to department rules. Practices teamwork and adheres to Standards of Behavior. Other Duties Performs other duties as assigned by management. Physical and Behavioral Requirements 

- Physical requirements include occasional stooping, reaching
