The LCT ensures a coordinated system for LCT case referral and tracking, maintaining a comprehensive resource database, collecting data and ensuring follow-up services.  The LCT Coordinator facilitates a coordinated approach to services and ensures parent involvement in LCT meetings. This position must foster strong internal and external relationships with local and State entities for the purpose of ensuring that children and youth with intensive needs and who are at risk of out of home placement receive culturally appropriate and comprehensive support services within the community. ESSENTIAL DUTIES AND JOB RESPONSIBILITIES

Plan, convene, manage and provide technical and administrative support to monthly and emergency LCT meetings; facilitate both in-person and virtual meetings; set agendas, track attendance, and document meeting notes Receive, complete and disseminate (through encryption) case referrals, recommendation summaries and other documents to the LCT members needed for family care plans Review and evaluate incoming case referrals from direct service agencies and the public to ensure appropriate. If not appropriate for LCT review, then provide community resources to referrals source and/or family Complete intake process with family to gather additional information as needed for the LCT  Follow-up with families and lead agencies to ensure action plan has been established and followed Administer on-line satisfaction surveys to families; analyze survey data to identify effectiveness of team, service gaps and develop strategies for improvement Track progress/updates on all Voluntary Placement Agreement (VPA) referrals; report to the State when requested Research and verify resources available for families with children entering or returning from placement; share knowledge of resources with various agencies on a system-wide, individual or child level Oversee and ensure LCT is comprised of appropriate agency representatives as required by Maryland Code, Human Services § 8-407; if/when there is an agency lacking representation, coordinate with respective agency to fill absence; maintain a current and accurate list of LCT members and points of contact for the LCT Designate and train newly appointed LCT Chair and Co-Chair as necessary Compose and send written correspondence on behalf of the LCT and Division Act as the primary LCT representative on behalf of the LMB and Division Establish and grow partnerships with local community partners Collaborate with LCT partners and other community partners to discuss and problem solve needs and gaps for children and families. Coordinate, review and revise the LCT policies and procedures to ensure compliance with County and State policies Coordinate with the State LCT staff person regarding technical assistance and guidance as needed Develop and maintain a comprehensive resource database/guide to include a directory of community-based resources. Disseminate resource guide to families seeking child, youth, and family related information and resources/services Oversee marketing for the LCT; create and distribute marketing materials; attend resource fairs to share information about the LCT Research and distribute new and/or upcoming child, youth, and family resources, best practices and emerging trends with LCT and LMB members, community partners, and families as appropriate Plan regular learning and networking opportunities for LCT members and community partners Oversee marketing for the LCT; create and distribute marketing materials; attend resource fairs to share information about the LCT Maintain and update LCT webpage within the DFS website Oversee and advise budget items within the DFS LMB program budget Participate in community meetings that align with the goals of the LCT Incorporate diversity, equity, inclusion and belonging (DEIB) into the work of the LCT Act as LMB staff and aid in facilitation of monthly meetings and community planning process Participate in the work of the LMB DEIB workgroup, which includes LMB members and community partners Maintain HIPAA compliance  Act as holder of LCT records and work with DFS and County Attorney when subpoenas for LCT documents are received Collaborate with LCT Coordinators in other jurisdictions Respond to community calls and email inquiries received by the division that require high level community service linkage  Represent the division and LMB at meetings, trainings, and workshops as assigned Oversee LCT flex funds procedures, including updating policy, reviewing applications, approving/denying financial requests, tracking in Excel spreadsheet; working with vendors to make payment and administer funds Use Excel spreadsheets to compile and analyze data and prepare reports to the LCT, LMB and the State on additional data not captured within the Scorecard, such as demographics, trends, challenges, and needs identified by the LCT Use LCT data to identify underserved areas in the County and provide presentations and attend outreach activities in those areas to increase knowledge and utilization of LCT Utilize Results Based Accountability (RBA) to analyze and report LCT performance measures to the State through the Scorecard data system.
