Overview: The Certified Nursing Assistant works as part of an interdisciplinary team to provide full range of personal care through regular visits in patient's home and in institutions. This position assists in the development and implementation of the patient’s Plan of Care and works in an interdisciplinary care setting to teach, assist and support patient/family with the dying process. Location: Orange Park
The Orange Park Care Center is Haven’s fifth free-standing care center serving north Florida is a state-of-the-art facility that allows us to expand our services to better meet the end-of-life and palliative care needs of patients and families in the area. The care center is located at 745 Blanding Blvd, Orange Park.
