Job Summary

As a Coordinated Family Support (CFS) Specialist at Premier Support services, you'll play a crucial role in assisting individuals and families by coordinating various services and supports that enable participants to maintain their residence within their family home. Your daily tasks will include:

Responsibilities

Developing skills within the home environment to foster independence and growth.

Conducting the initial CFS assessment of the participant and family.

Preparing Individual Service Plan (ISP) based on reports of discovery, inputs from participant and the family.

Review minutes of meetings between CFS workers and the individual served to ensure the effectiveness of service plan and adjust as needed.

Administer a quarterly performance evaluation survey on the participant and family.

Helping in accessing community resources and services.

Coordinating the delivery of necessary services to meet individual needs.

Offering information and resources pertinent to the individual's diagnosis and required supports.

Managing scheduling for medical, educational, vocational, and other appointments as needed.

Write Quarterly report to program director on the quarterly performance survey, including any remedial actions where necessary.

Identifying transportation options or services for individuals.

Assisting with future planning initiatives.

Overseeing participant’s entire delivery service to ensure effectiveness.

Additionally, you will facilitate activities and routines both at home and within the community to promote stability and permanency for the individuals under your care. Maintaining accurate documentation of interactions, progress, and relevant information is essential to this role.

Qualifications

Must have a bachelor’s degree in a human services field of study or Associate’s degree in a human services field of study and 3 years of experience in working with people with developmental, cognitive, or physical disabilities.

Excellent communication and interpersonal skills are essential.  The ability to work independently, demonstrate problem-solving skills, and maintain composure in challenging situations is crucial.

Understanding of mental health and developmental disabilities is preferred.

Strong adherence to confidentiality and ethical guidelines is expected.

Proficiency in English and Spanish language (reading, writing, speaking) is highly desirable.

Must possess a reliable vehicle, a valid driver's license, and car insurance.

Successful completion of a criminal background check is mandatory.

Must have very strong office MANAGEMENT skills as well as CLERICAL and WRITING

Bilingual English / Spanish is Required Able to Commute Valid Drivers License (Required) Los Angeles Area Associates Degree 3 Year Experience

Work Location: We have a combination of remote and In-person service delivery. We service participants in the Westside Regional Center catchment area (Calabasas, Malibu, Topanga Canyon, Pacific Palisades, Bel Air, Los Angeles, West Los Angeles, Beverly Hills, Santa Monica, Venice, Marina Del Rey, Playa Del Rey, Playa Vista, Westchester, El Segundo, Mar Vista, Culver City, Inglewood, Gardena, Lennox, Lawndale, Hawthorne)
