Duties

Duties:

Demonstrates proficiency in practice based on conscious, deliberate planning. Self-directed in goal setting for managing complex patient situations.

The RN Case Manager facilitates care coordination and navigation through a complex health care system.

The RN Case Manager uses the nursing process to assist in population management, a data-driven process for proactively defining a cohort of patient who might benefits from a health care plan or reaching out to individual patients in the cohort to offer the right interventions at the right Lime, rather than waiting for the patient to self-identify and seek out health care.

The Nurse Manager, as immediate supervisor, provides supervision and counseling, approves scheduled leave, and evaluates performance.

Responsible for completion of assignments given by the direct supervisor using initiative in carrying out care with referral of unusual problems to the physician.

Meets the needs of customers while supporting VA missions. Consistently communicates and treats customers (veterans, their representatives, visitors, and all VA staff) in a courteous, tactful, and respectful manner.

Provides the customer with consistent information according to established policies and procedures.

Handles conflict and problems in dealing with the customer constructively and appropriately.
