The RN Case Manager integrates and augments health care quality by providing health education, clinical appropriate preventative and therapeutic care, and partnership among patients, personal support persons and staff.
Duties
Duties:
The RN Case Manager facilitates care coordination and navigation through a complex health care system. It involves working across multiple care settings, to include accessing health care providers and other services in the community to help patients receive the care they need and want without the unnecessary duplication of serviced or avoidable inconvenience. Care Coordination involves open communication among health care providers, legally permissible exchange of health care information, and logistical integration of health care encounters. The RN Case Manager uses the nursing process to assist in population management, a data-driven process for proactively defining a cohort of patient who might benefits from a health care plan or reaching out to individual patients in the cohort to offer the right interventions at the right Lime, rather than waiting for the patient to self-identify and seek out health care.
