Job Overview - Care Delivery Supervisor (RN):
Join our client's team as a Care Delivery Supervisor in New York, NY, where you will oversee clinical services provided at participants' homes. Compensation: $66/hour
Location: New York, NY
Schedule: 8:30 AM to 5:00 PM, Monday to Friday

Responsibilities as the Care Delivery Supervisor (RN):
  • Supervision: Delegate responsibilities and supervise personnel, ensuring quality care and compliance with care plans.
  • Health Assessment: Evaluate the healthcare needs of participants, considering physical, mental, and environmental factors.
  • Guidance: Supervise and guide Community Health Nurses (CHNs), Home Health Aides (HHAs), and other personnel in delivering home health care services.
  • Documentation: Complete and maintain all required documentation, ensuring it aligns with federal and state regulations.
  • Education: Provide education to participants and aides on medications, safety, and diet.
  • Field Visits: Conduct field visits, provide supervision, and support to CHNs and HHAs as needed.
Qualifications for the Care Delivery Supervisor (RN):
  • Education: Degree from an NLN-accredited nursing school (RN, BSN required).
  • Experience: 1+ years in home care as a community health nurse within a Certified Home Health Agency or similar setting (experience with frail or elderly populations preferred).
  • Licensure: Current active and unrestricted RN license in New York State.
  • Other Requirements: Reliable transportation, valid NYS driver s license preferred, medically cleared for communicable diseases, and up-to-date immunizations.
