The Residential Service Coordinator will coordinate and link residents to needed services that will assist them to age in place. Provide information and make appropriate referrals to residents and families in all areas of case management for services delivered at Clairmont Oaks.

Responsibilities:

  • Locate, coordinate, and monitor residential and community services for residents.
    • Perform psycho-social assessments as necessary in order to make recommendations and referrals to residents and their families for services.
    • Conduct one-on-one visits with new residents to explain the Service Coordinator program and perform initial psycho-social assessments.
    • Attend departmental meetings and other resident-related meetings (Senate), as directed by supervisor, to ensure that the best interest of the residents is served in all plans at Clairmont Oaks.
      • Stay up to date on all issues that affect senior adults, such as public benefits, social security, Medicare, etc. and keep staff and residents informed. Provide education and information to residents and families as requested to ensure that they are informed consumers for all services.
      • Plan and implement informational and educational programs for the Clairmont Oaks community.
      • Assist in producing a monthly "Service Coordinator Corner” page for The Oakleaf'     (newsletter).
      • Attend training workshops as needed to continue professional growth.
      • As directed by supervisor, assist with or manage the following responsibilities when needed: Resident Orientations, Resident Emergency Forms, Health-related studies conducted in the building.  
      • Review incident reports and resident referrals as assigned by supervisor and provide case management as needed regarding these reports.
      • Maintain contact and good professional relationships with physical therapists, clinical nurse specialists, hospital discharge planners, social workers, and other professionals at nursing and rehabilitation facilities.
      • Coordinate plans and services for residents returning from hospitals or other facilities. Coordinate plans and services for assigned residents returning from hospitals or other facilities. Address walk-in traffic and field questions and problems in hallways and common areas of the building.
      • Assist other staff as needed in responding to emergency calls in the building.
      • Assist with fire drills and other emergency plans and evacuations.
      • Perform all other duties as assigned.

        QUALIFICATIONS:

        • Minimum of a Bachelor's degree in one of the social sciences.
          • Education in gerontology and/or experience working with older adults.
            • Excellent case management skills.
              • Ability and experience in interviewing and assessment techniques and skills
                • Ability and experience in assessing psychosocial needs of the elderly, the provision of social services to help meet those needs, and the skills to monitor and document the delivery of the services.
                  • Ability and temperament to respond to emergency situations and to make clear decisions regarding residents' health and safety in these type situations.
                    • Ability to work as a member of a team.
                      • Excellent interpersonal skills and good communication skills, both verbal and written.
                        • Knowledge of community resources available and the skills to access them and /or help the residents access them.
