The Social Worker assigned to the MHRRTP provides clinical psychosocial and case management services at an independent practice level. He/she should have clinical knowledge of substance use and mental health diagnosis; must be able to complete assessments, have clinical understanding of complex diverse populations, and work independently with little oversight and supervision. Assist Veterans through the process of admission, treatment, discharge and aftercare phases and must have clinical knowledge of co-occurring disorders.

The incumbent tasks include, but are not limited to:

- Interviews Veterans referred to the MHRRTP to establish facts about the Veteran's situation, presenting problems and their causes, and the impact of such problems on the Veteran's functioning and health as part of a comprehensive psychosocial assessment. Manages and addresses each application, responds to agencies and/or colleagues referring Veterans to the MHRRTP. Assists Veterans with the enrollment process as necessary. Responds to Veterans who may walk-in to the program for information and admission. Initiates contact with Veterans to schedule screening assessments and follows up with applicants throughout the screening process. This may consist of multiple contacts with Veterans with complex psychosocial issues.

- Is responsible for assist in the development of psychosocial treatment plans in coordination with MHRRTP Team members, including goals for psychosocial clinical treatment.

- The incumbent is the subject matter expert on VA and/or community resources. The social worker will collaborate with other service providers in reassessing the veteran's needs for non-institutional, institutional services/programs and entitlements. The incumbent is responsible for educating the veterans and/or caregiver of the available services and assisting them in establishing the appropriate referrals based on the veteran's preference or that of his surrogate decision-maker.

- Responsible for developing, updating and using current social worker resources within VA and community social service programs.

- Acts as an advocate when it serves the best interest of the veteran/family members and/or caregivers.

- Will conduct crisis assessment and develop a crisis management plan. Also, will be able to identify safety issues for high-risk patients' populations establishing the appropriate clinical services and referrals.

- During the response of an emergency, the social worker is considered essential personnel and will be ready to assist in any capacity given by the Incident Commander.

- Provides psychoeducation to the Veteran, family members/caregiver and refers them to appropriate interdisciplinary team members or services for identified health education needs. Additionally, the social worker provides information about VA benefits and refers to the appropriate service organization for assistance. Assists with the orientation process for new MHRRTP residents. provides on-going case management to MHRRTP residents. Assists MHRRTP residents with concrete processing of needs assessment in order to begin planning realistic steps necessary for housing and employment upon discharge from the MHRRTP. Provides assistance with psychosocial issues which may prevent a resident from successfully reintegrating into the community.

- Will educate the veteran, their families and the team of all the options available to them and will collaborate with the veteran and family on the preferred options.

- Will provide the veterans and their caregivers with ongoing counseling.

- Will enter all Veteran/family contact with the Veterans' written and verbal consent in the electronic records system (CPRS).

- Will establish and maintain positive, professional working relationships.

- Will collaborate in the performance improvement processes and comply with performance measures as required by the VA for their specific population and/or program.

- Is responsible for his/her own professional growth through continued education for ensuring continued education.

- Is responsible to maintain a level of productivity and quality consistent with the complexity of the assignment, clinic panels and consistent with Social Work Standards, Joint Commission and facility Standards.

Work Schedule: 8:00am - 4:30pm, Tuesday to Saturday OR Sunday to Thursday, may include some evenings, and holidays (Note: The incumbent may be required to work various shifts on a permanent or temporary basis based on facility/patient care needs).
