The social worker in this position provides social work services to the Veteran population enrolled in the Mental Health Intensive Case Management Program.
The MHICM Social Worker serves Veterans with Severe and Persistent Mental Illness (SMI) who have frequent or long-term hospitalizations.
The MHICM Social Worker will meet with veterans and their families in their home and in their communities.
The MHICM Social Worker provides clinical case management to Veterans who have illnesses which are chronic in nature, refractory to resolution, and requires close monitoring to maintain stability, while encouraging Veterans of the hope of Recovery.
The MHICM Social Worker will individually tailor services to meet the needs of the Veteran, assuring continuity of care by using a team approach and sharing responsibilities.
At least 75% of the service is provided to the Veterans in the community.
The program is based on the evidenced-based Assertive Community Treatment model which has proved effective with the SMI population.
The MHICM Social Worker performs psychosocial assessments, diagnosis and treatment planning (recovery plans), individual, family and group psychotherapy.
Collaborates with a multidisciplinary team to provide advanced level case management services, coordination of care, referrals, advocacy, psychoeducation, transition, and discharge planning for Veterans admitted to the MHICM program.
The incumbent will share responsibility for managing the after-hours crisis phone.
The incumbent will follow all guidance concerning completion of all documentation.
The incumbent will manage all aspects of their assigned caseload and also assist other team members, as needed.
Preferred Experience: (1) year of experience working with patients diagnosed with Serious Mental Illness.
Work Schedule: Monday - Friday, 7:30 a.m.-4:00 p.m. OR 8:00 a.m.-4:30 p.m. work schedule, but work schedule may be adjusted to meet the needs of the service.
