Advocate Health in Oshkosh, Wisconsin is seeking a primary care physician who enjoys nursing home care to grow our geriatric program as a **Skilled Nursing Facility (SNF) Physician**. This position is part of a stable health care system that is committed to achieving high quality health care.

- Join a team of two physicians and two NP’s in our SNF’ist program in Oshkosh and Fond du Lac
- Incoming clinician will primarily be covering three facilities in Oshkosh
- Stable health care system in top decile for quality of patient care
- Physician-led medical group with established primary care service lines
