**Job Description: Care Management Support Specialist**

**Position Purpose:**

The Care Management Support Specialist plays a vital role in ensuring that care management activities are effectively executed, with a focus on delivering quality services, maintaining continuity of care, and achieving high member satisfaction. This position involves engaging with members through telephonic outreach or home visits to support care plans and service coordination, as well as addressing inquiries and documenting care activities.

**Key Responsibilities:**

- **Member Outreach:** Perform outreach to members via phone or home visits to discuss and clarify care plans/service plans, including next steps, available resources, and address any questions or concerns. Provide ongoing education to members as appropriate throughout their care journey.

- **Care Coordination:** Coordinate care activities according to the care/service plan, collaborating with healthcare providers, community partners, and members/caregivers to accommodate any changes or progress in care needs.

- **Support and Communication:** Address various member and provider inquiries related to care/service plans. Communicate with care managers, practitioners, and other relevant parties to facilitate member services and ensure continuity of care.

- **Service Assessments:** Support the performance of service assessments and screenings for members, documenting their care needs and updates as required.

- **Record Maintenance:** Document and maintain member records in compliance with state and regulatory requirements. Ensure records are accurately distributed to providers as needed.

- **Policy Compliance:** Adhere to standards of practice, policies, and regulatory guidelines, ensuring compliance with contractual and regulatory requirements.

- **Referrals and Education:** Identify member needs and make appropriate referrals to Care Managers, community organizations, and Disease Managers. Provide education on benefits and available resources.

- **Additional Duties:** Perform other duties as assigned to support the care management process.

**Education/Experience:**

- **Education:** High School diploma or GED required.
- **Experience:** 1-2 years of related experience in a call center environment with familiarity in medical terminology.

**Required Skills:**

- Proven ability to engage with members effectively and provide clear explanations of care plans.
- Strong coordination skills to manage care activities and communicate effectively with various stakeholders.
- Experience in handling inquiries, documenting care needs, and maintaining accurate records.
- Knowledge of policies, standards, and regulatory requirements relevant to care management.
- Ability to perform outreach, assessments, and referrals efficiently.
- Proficiency in providing education on benefits and resources.
