Residential Coordinator Job Description

What is your role as a Residential Coordinator?

The Day Coordinator supports the current Shared Living Provider. This position provides monitoring and support to those in residential services to ensure Shared Living Providers deliver the highest level of quality services possible, while maintaining compliance with the Individual Service Program- ISP.

Responsibilities:

  • providing feedback, support, and guidance on caring for an individual residing in their home.
  • Supervision and support the provider through monthly home visits to ensure fidelity of programming. Includes additional training and remediation as needed.
  • Performing and coordinating intake of new clients.
  • Development of treatment plans by conducting behavioral and skill assessment of clients though behavioral assessments to identify and access individual strengths and needs.
  • Function as a member of a treatment team for both adults and children in the system. This includes working with a Therapists, guardians, physicians, DHHS workers,. Work in collaboration with schools, providers, mental health professionals, and other community resources.
  • Completing monitoring activities and providing technical assistance to ensure the accurate and timely completion of required documentation
  • When on call, provide timely responses for off hours crisis management
  • Exception funding – will be reviewed by our QA/QI before submission
  • Program writing – will be reviewed by our QA/QI before implementation
  • MGR – will be reviewed by our QA/QI for accuracy
  • Intake collection/meeting – will be reviewed by our QA/QI for accuracy
  • Other tasks assigned by administration team if applicable

Daily Job Duties

  • Email and phone messages
  • Therap: Ensure contractors are completing daily data (MAR, Side effect log, ISP Programs, T-Logs, S Comms, GER, and RRC Forms. Contractor should never be more than 2 hours behind or 2 hours for the MAR.
  • Read T-logs
  • Review, Edit and Save GERs
  • Attend Home Visits and meetings
  • Work along with finance for any needs of the individual or for authorizations
  • Work along with HR and training to assist in communications with background checks or training requirements
  • Review any SComms in Therap
  • Weekly Job Duties
  • Thursday Treatment team meeting at 12:00 pm in the conference room

Monthly Job Duties

  • Data Validation (Checking to ensure data is completed by SLP)
  • Complete and audit of client’s progress in their program (MGR)
  • Complete a home visit with your individuals
  • Document any visits/meetings you made for the month on event documentation form and audit form.
  • Prepare for any ISP meetings

Yearly Duties

  • Attend and prepare for all ISP meetings

Your Residential Team is always your 1st line of contact. Whether your director, a lead, or a peer, in most cases, your team can assist you with any questions you have.

As a coordinator, you are considered in a middle management position. It is a requirement that you remain professional in this role, in all your interactions with others in the office, upper management, and those in the community. Professionalism Expectations include:
