This position enables VA homeless programs to fully collaborate with the community in the coordinated entry efforts in that community. Responsible for outreach services to homeless veterans in communities and rural areas. Has experience that demonstrates possession of master level practice skills and judgement in social work services for homeless Veterans. Supports the VA Homeless Program by providing continuity of care for homeless Veterans between VHA and community-based programs, as the Veteran transitions from homelessness into programs. Provides leadership in the community by actively participating in the local plans to end Veteran homelessness as well as conducting and leading the coordination of outreach services to homeless Veterans. Must be able to develop appropriate professional relationships with community stakeholders, and is responsible for communicating potential CoC policy or protocol changes to local VA leadership. Facilitate VA involvement in the CoC's Coordinated Entry System (CES) and will be responsible for helping VA meet federal guidelines on CES. Needs to coordinate their efforts with local planning and support currently provided by VA, HUD, and USICH technical assistance initiatives. Also function as a liaison, participating in case conferencing and other pertinent community meetings. Is the VA liaison for coordinating outreach efforts with local community partners.

Functions

Participates in CoC meetings and planning efforts
Provides direct linkage from community services to VHA homeless program services through regular and consistent outreach and communication with community providers
Participates in case conferencing process to help inform and facilitate referrals to VA programs
Ensures efficient sharing of Veteran data and program information
Participates and contributes to a CoC level resource-and-demand analysis and make recommendations to VHA homeless program leadership on adjustments to resource allocations
Provides support to the VA homeless program teams as a functional member of the team
Serves as a member of a multidisciplinary homeless program treatment team to link treatment team discussions to the community's case conferencing discussions
Provides all necessary assessment functions in the service provision for homeless Veterans
Develops partnerships with community agencies with regular contact and communication
Participates in policy formulation with federal partners
Provides appropriate clinical documentation for all contacts with or about Veterans who are engaged through outreach services
Provides clinical services and appropriate clinical documentation for homeless Veterans
Ensures timely and accurate entry into VA data systems and review the data to improve performance and delivery of services to homeless Veterans and ensuring that this information is shared so that community Master or By-Name Lists are up-to-date and complete
Provides recovery-oriented and housing first services

Pay:
Competitive salary and regular salary increases When setting pay, a higher step rate of the appropriate grade may be determined after consideration of existing pay, higher or unique qualifications, or special needs of the VA (Above Minimum Rate of the Grade).


Paid Time Off:
37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)

Parental Leave:
