Community Health Worker Overview
Key responsibilities include, but are not limited to:
- Promote wellness by providing culturally appropriate health information to patients and providers while educating patients on disease prevention, self-management of chronic illnesses and medication adherence
- Bridge the gap between communities and the health and social service systems by enhancing care quality, aiding communication between health system(s) and patient to clarify cultural practices and health information
- Increase access to primary care through culturally competent outreach and enrollment strategies, making referrals and coordinating services to build/enhance patients' knowledge, skills, and confidence to access resources and obtain care
- Enroll patients into programs such as health insurance and public assistance
- Build Individual and Community Capacity
Minimum Qualifications:
- Prior experience as a community health worker, peer specialist, care manager assistant or similar.
- Experience, competence and sensitivity in working with diverse communities.
- Fluent in English.
- Current Driver's License

Hours: Full Time, 40 hours a week, weekday day time hours
