Accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal care.
Provides gender-specific direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department.
Performs triage and assessment of patients on the team panel or unit making sure to provide prompt and appropriate treatment for Veterans health concerns to ensure care is timely.
Provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management.
Manage and monitor high risk patients on the panel for improvement of patient outcomes including prevention of hospitalization and improvement in ambulatory care sensitive conditions.
Collaborates and coordinates care with interdisciplinary team members from other services within the department as well as services in the community.
Formulate a plan of care based on nursing assessments. Seeks guidance on how to improve care provided to patients and families.
Delegates elements of care to the appropriate individual based upon assessed needs, condition, and safety of the patient.

Preferred Experience
: 2 years recent Med Surg / ICU / ED experience/Primary Care Outpatient.
