This position is eligible for a \$10,000 recruitment bonus. What You Will Do: The RN Case Manager works collaboratively with patients,social workers, physicians, other practitioners, caregivers and community agencies in coordinating and overseeing the interdisciplinary plan of care for a patient in a home care/home hospice environment. Assesses, plans, facilitates and evaluates timely coordination of high quality, cost-effective care. Responsible for improving the health of patients dwelling in their own homes.
