RN Case Manager

The RN Case Manager plays a vital role in coordinating and overseeing the interdisciplinary plan of care for patients in a home care/home hospice environment. This position requires a nurse who is comfortable working independently, enjoys teaching, and possesses strong communication skills.
Key Responsibilities- Assess, plan, facilitate, and evaluate timely coordination of high-quality, cost-effective care.
- Work collaboratively with patients, social workers, physicians, other practitioners, caregivers, and community agencies to ensure seamless care delivery.
- Provide hands-on care in the home environment and offer autonomy, flexibility, and the luxury of caring for one patient at a time.
- Promote safety, cost efficiency, and a commitment to quality improvement.
- Associate's Degree in Nursing required; Bachelor's Degree in Nursing preferred.
- Current State of WI or eNLC compact state RN license.
- Valid State of WI driver's license with an acceptable driving record.
- Current BLS (CPR) certification or ability to obtain upon hire.
- Clinical experience preferred.
- EPIC software experience preferred.
