St. Vincent’s Medical Center in Bridgeport has more than 3,200 employees. St. Vincent’s is the first hospital in Fairfield County to be integrated into the Hartford HealthCare network, and is the system’s second-largest hospital.

Job Summary:

The Nurse Navigator plans, coordinates and delivers health services to patients receiving cranial neurosurgical care. This includes working collaboratively with other health care providers to assure coordinated care throughout the continuum of care. The nurse navigator will assist with the coordination of services and will provide support and education throughout the clinical course. They will serve as a professional resource for the program.


Responsibilities:

· Acts as a liaison between healthcare providers, payers, and the patient. Assists with the scheduling and coordination of appointments related to the patient’s work-up, surgery, and follow up care.

· Serves as a consistent contact for the patient and family pre and post-operatively. Provide education to patients and families regarding disease process, treatment plans and prevention throughout their care.

· Works with providers and members of the care team on treatment plans and follow up care. Attends case conferences and program specific meetings.

· Collects and manages data for patient tracking and outcomes. Participates in outcomes research.

· Identifies opportunities to continually improve processes pertaining to the program coordination of care.

· Facilitates referrals by collaborating with referring providers.

Qualifications

· BSN

 

Licensure, Certification, Registration:

· Licensed as a professional RN in the state of Connecticut.

· Certification in specialty area.

 

Work Experience:

·5 years nursing experience. Experience within neurosurgery. Experience in a multi-specialty environment.

· Highly effective verbal and written communication skills.

· Ability to work effectively with other departments.

· Computer literate and database managing experience

· Ability to manage, organize, prioritize and delegate multiple responsibilities

· Ability to work independently

· Strong interpersonal skills
