*The primary purpose of the facility-based Certified Wound Care Nurse Practitioner is to provide patients with quality wound care, chronic medical conditions, and comprehensive medical care within the Post-Acute Care setting.
*Performing physical, wound care exams & patient observations
* Creating patient care plans & analyzing diagnostic tests.
* Diagnosing health issues.
* Consulting with healthcare professionals and families.
* Detecting changes in patient's health & modifying treatment, as needed.
* Performing Wound Care treatment.
* Valid nurse practitioner license in state of operations & current board certification in your field
* Masters Degree in Nursing
* Wound Care Certification
