The Cardiologist partners with our team-based care model to support our value-based care provider focused on quality of care for the patients we serve. practice professionals, medical assistants, behavioral health support, quality-based coders, referral coordinators and more. Our approach allows us to provide an unmatched experience for seniors. model is positioned to provide higher quality care and better outcomes for seniors by providing concierge experience, multidisciplinary services, coordinated care supported by analytics and tools, and deep community relationships. Assists in the coordination of patient services, including but not limited to specialty referrals when appropriate, cardiology subspecialists, and vascular surgeons, etc.
Acts as an active participant and key source of medical expertise with the care team.
Completes all medical record documentation daily working with a quality- based coder to optimize comprehensive documentation.
Follows policy and protocol defined by Clinical Leadership.
Meets with primary care physicians (including nurse practitioners) and market medical director.
and about quality of care, review of outcome data, and medical records issues.
Clinically focused on direct patient care, inclusive of patient facing time and general administrative time (charting, meetings, etc.) Support readmission reduction initiatives for heart failure and CAD.
Help coordinate care for CKD stage 5 patients needing AV graft placement prior to initiating hemodialysis.
Interpret and document in detail test results to determine how effectively the heart and or cardiovascular system is functioning i.e., Coordinate patient care with cardiology subspecialist i.e., Graduate of accredited MD or DO program of accredited university.
Current and unrestricted medical license or willing to obtain a medical license in state of practice; Board Certification Cardiology
Basic computer skills, including email, EMR, and
Knowledge of HEDIS/STARS
Experience working in medically underserved/culturally diverse communities.
Experience working with an electronic health record.
Experience working in Medicare Advantage Global Risk Models.

Reports To: Network Manage.
