The Clinical Program Coordinator for Population Health will be responsible for developing, maintaining and aligning SNF collaboratives for both Tri-Cities and Walla Walla. ~ Full-time, 1 FTE, Day Shift, 0040-No Daily Overtime 40 Hour Week
Associate's Degree From an accredited nursing program
~ 2 years in an acute health care hospital/integrated health system
5 years in an acute health care hospital/integrated health system
~1 year experience within a skilled nursing facility or home health
