RN - Clinical Program Coordinator - Kadlec Regional Medical Center - Richland, WA
The Clinical Program Coordinator for Population Health will be responsible for developing, maintaining and aligning SNF collaboratives for both Tri-Cities and Walla Walla. The primary focus within the collaborative will be SNF utilization, length of stay and readmission. The Clinical Program Coordinator will be responsible for facilitating positive, collaborative business interaction with key stakeholders and affiliates within the aligned SNF collaboratives and improving patient outcomes. This is accomplished by relationship development, coordination, and prompt response to stakeholder requests.

Shift Details :
- Full-time, 1 FTE, Day Shift, 0040-No Daily Overtime 40 Hour Week
Required qualifications:
- Associate's Degree From an accredited nursing program
- Upon hire: Washington Registered Nurse License
- Upon hire: National Provider BLS - American Heart Association
- 2 years in an acute health care hospital/integrated health system
Preferred qualifications:
- 5 years in an acute health care hospital/integrated health system
- 1 year experience within a skilled nursing facility or home health
