The Social Worker would provide assessments, direct care, and facilitation of referrals to individuals who are struggling with mental illness, substance use disorder, or a dual diagnosis. Additionally, the Social Worker would provide support to the family members who are being impacted by their dependency. Health and Dental Insurance

The Social Worker will provide individual and group therapy to adults, adolescents, and children with the full range of DSM diagnoses, including both mental health and substance use disorders, in an acute care, outpatient setting.
Periodic, field-based work required.
Complete all clinical documentation in accordance with NYS Office of Mental Health in an electronic medical record (EMR).
The Social Worker will follow up with clients during the stabilization period and immediately following.
Attend routine supervisions and team meetings.
Complete training and maintain proficiency with de-escalation and physical restraint skills.
Master’s degree in social work required.
Proficient computer skills, including Microsoft Office and Electronic Health Records required.
Some evening and weekend hours may be required.
