Social Worker to provide clinical services to youth and families through our Children and Family Treatment and Support Services (CFTSS) program. CFTSS is a team-based model which is intended to provide families individualized and flexible services by meeting with children in their homes and other community settings. CFTSS provides treatment to children ages 5-21 and enables a greater focus on prevention and early intervention by providing an array of services and the capacity to intervene earlier in children’s lives.

The Social Worker will provide community based mental health treatment to a caseload of 20 children/youth and their families.
Deliver services with the core values of CFTSS: “Family-Focused, Community- Based, Multi-Systemic, Culturally Competent, and Least- Restrictive.”
The Social Worker will provide evidence-based interventions that are tailored to the child’s and family’s needs.
The Social Worker will complete all documentation in accordance with CFTSS standards in an electronic medical record, including notes, assessments, and treatment plans.
Collaborate with other CFTSS and Care Coordination providers by engaging in regular communication.
Master's degree in social work or related field required.
Prior experience diagnosing using the DSM V and experience with behavioral/therapeutic interventions for children, adolescents, and families required.
Proficient computer skills, including Microsoft Office and Electronic Medical Records (EMR) required.
