Recently named by The Boston Globe as one of the Top Places to Work in Massachusetts, our community based, mission-driven hospice and palliative care organization is now offering a Transition-to-Practice RN Resident designed to orient newly licensed nurses to the rewarding role of Hospice Nurse Care Coordinator. The TTP RN Resident will build confidence and skill through various learning modalities, guided experiences and extended preceptorships. Travel is required for this position. Care is provided in patient’s private homes and residential facilities throughout Greater Boston and parts of Metrowest.

Education, Skills and Experience:

Graduate of an accredited BSN or associates in nursing program

Successful completion of NCLEX exam

Strong interest in Hospice

Commitment to personal and professional growth

Basic computer proficiency

Job Responsibilities:

Essential Functions:

1. Attend didactic classroom sessions and other learning experiences

2. Engage in self reflection and professional growth activities

3. Participate in Good Shepherd Interdisciplinary Team meetings in accord with Good Shepherd guidelines.

2. Under the guidance and supervision of an NCC preceptor and TTP educators, learn to provide, coordinate, and document initial and ongoing clinical assessments of the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial and environmental needs and activities of daily living from referral through bereavement phase.

3. Expand communication skills including educating patients and families in regards to the disease process, self-care techniques, end-of-life care, and the processes for dealing with issues of ethical concern.

4. Learn appropriate documentation, including admission process, clinical progress notes,

recertification procedures and various disposition documentation.

5. Gain knowledge on ways to manage patients’ care in a clinically and fiscally

responsible manner.

6. Under the guidance and supervision of NCC preceptor and TTP educators,

learn to consult, collaborate, and coordinate patient care with various health care facilities

and professional health care providers, communicating with the patient's internal and external care team about changes in the patient’s condition and needs as well as outcomes of interventions.

7. Learn to determine scope and frequency of services needed based on acuity and

patient/family needs, identify need for additional personnel, e.g. home care aides, and learn

to supervise such personnel on a case by case basis and according to regulatory and

agency guidelines.

8. Participate in in-service programs as designated by the supervisor.

9. Learn to communicate information using Good Shepherd’s current process.

10. Participate in other activities as designated by Senior Nursing Manager, Clinical Educator and NCC preceptor.
