QMHP-A - Emergency Services Care Coordinator

Hours: full-time (37.5 hours per week), +10% of salary emergency services stipend
The Emergency Services Care Coordinator will serve within Horizon's emergency services team to support the Department of Behavioral Health and Disability Services' (DBHDS) requirements for collaborative discharges for clients admitted to State hospital facilities. In addition, this position will provide similar discharge coordination services to high utilizers of emergency rooms and private hospital admissions.
Bachelor's degree in psychology, social work, counseling or human services field
Knowledge of severe mental illness, trauma informed care, and substance abuse disorders; Ability to establish strong therapeutic relationships with clients and their support network; Ability to manage multiple cases simultaneously along with the ability to handle sudden changes in a clients status or clinical discharge plan; Ability to communicate effectively verbally and in writing in order to facilitate treatment and relay information to others as part of an interdisciplinary team; Ability to promote positive, collaborative, and professional working relations among co-workers and representative in the community; Ability to participate in and successfully pass behavioral intervention training; Willing and available to work modified schedules (weekends, holidays, etc.) May be called upon to travel to any Horizon worksite; transport clients, provide services to client @other locations. Regular attendance at trainings and Horizon meetings may require travel to a variety of Horizon sites.
