* The Case Manager integrates the roles of Utilization Management, Case Management and Discharge Planning. The job objective is to promote a collaborative interdisciplinary care delivery model to achieve optimal clinical and financial outcomes with adherence to regulations and compliance.
Reports to: Director of Quality/Risk/Patient Safety
* Perform initial and ongoing discharge screens/evaluations to determine general health behaviors, practices and resources that will influence the patient and family's abilities to optimally manage chronic conditions.
* Assess emotional status and quality of relationships and adequacy of social support systems, identifying new sources of support.
* Prioritizes immediate plan for barriers to discharge including medication availability for use, shelter and appropriate food, primary care physician coordinating care resources accordingly
* Maintains clinical and financial documentation
* Participates in performance improvement processes as well as other duties as required by the job
Associates/Bachelor's degree in Nursing
Case Management Certification is preferred
Ability to understand, communicate, and react effectively to patient and family needs
* Must be able to work collaboratively with internal and external resources for optimal discharge planning
