Work schedule is 2 - 3 week days plus every other weekend** CVS Health is seeking a Nurse Practitioner (NP) to join our Primary Care team. In this capacity, you will work in collaboration with a dedicated clinical care team to provide evidenced-based care to a panel of patients. The clinical care team will work with the provider daily to drive better outcomes for entire panel of patients. This will include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning. This role will report to the practice manager. *PRIMARY DUTIES AND RESPONSIBILITIES: * Responsibilities include but are not limited to
- Patient-Centered Quality and Safety*
- Uses knowledge of etiologies, risk factors, pathophysiology, presentation, and epidemiology of medical conditions relevant to primary, acute, chronic, and transitional to provide care to patients
- Performs history and physical assessments of patients that are accurate, complete, concise, relevant, and appropriate for age, gender, and clinical symptoms.
- Provides patient counseling; inclusive of pregnancy prevention, STI Prevention/safer sex practices, contraceptive care counseling and medication management
- Counsels and educates patients and families on health maintenance and respond to patient care inquiries
- Evaluates and treats patients' medical needs with safe and high-quality treatment
- Documents all patient care within an electronic health record in accordance with current policies and procedures
- Works as a member of the clinical care team providing care and coordination of our patients with internal and external colleagues ensuring the highest level of care is always provided for all patients
- Provides patient-centered care that is compassionate, appropriate, and effective for the promotion of health, prevention of illness and treatment of disease
- Refers to appropriate specialists and patient care components as appropriate
- Maintain patient confidentiality in accordance with PHI and HIPAA standards

- Required Experience:*
- A minimum of 1 year experience as a Nurse Practitioner is required. Experience in Family medicine or primary care practice preferred.
- Internal applicants with less than 1 year experience are eligible with successful completion of the CVS Minute Clinic Primary Care Training Program
- Active Advance Practice Registered Nurse, Nurse Practitioner license in good standing
- Current National Board Certification in Family Practice
- Working knowledge and understanding of quality measurement in the management of chronic disease conditions.
- Self- motivated, prioritizes and solves problems, takes initiative, and advocates for their patients and their practice
- Effective verbal, written, and electronic communication skills

- While performing the duties of the job, the Primary Care NP is regularly required to interact with patients remotely, sit, write, operate the computer and phone, speak intelligibly, and hear patient responses
