In this capacity, you will work in collaboration with a dedicated clinical care team to provide evidenced-based care to a panel of patients. The clinical care team will work with the provider daily to drive better outcomes for entire panel of patients. This will include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning. This role will report to the practice manager. *PRIMARY DUTIES AND RESPONSIBILITIES: * Responsibilities include but are not limited to

  • Uses knowledge of etiologies, risk factors, pathophysiology, presentation, and epidemiology of medical conditions relevant to primary, acute, chronic, and transitional to provide care to patients
  • Performs history and physical assessments of patients that are accurate, complete, concise, relevant, and appropriate for age, gender, and clinical symptoms.
  • Provides patient counseling; inclusive of pregnancy prevention, STI Prevention/safer sex practices, contraceptive care counseling and medication management
  • Counsels and educates patients and families on health maintenance and respond to patient care inquiries
  • Evaluates and treats patients' medical needs with safe and high-quality treatment
  • Documents all patient care within an electronic health record in accordance with current policies and procedures

*EDUCATION: * Completion of master’s degree level Family Nurse Practitioner with current National Board Certification and State of Employment license to practice in the Advanced Practice role required.
  • Required Experience:*
  • A minimum of 1 year experience as a Nurse Practitioner is required. Experience in Family medicine or primary care practice preferred.
  • Internal applicants with less than 1 year experience are eligible with successful completion of the CVS Minute Clinic Primary Care Training Program
  • Active Advance Practice Registered Nurse, Nurse Practitioner license in good standing
  • Current National Board Certification in Family Practice

  • Pay Range*
The typical pay range for this role is: $41.85 - $90.12This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
